FOR CHILDREN UNDER 18: I acknowledge that my child may participate in a variety of activities provided at the Arts Council WF and I represent that he or she is physically fit to do so. I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which might be incurred as a result of participating in the program.
FOR CHILDREN UNDER 18: I give permission for my child to be photographed for publicity and/or archival purposes, including but not limited to use on the Arts Council's website, social media site(s), print and electronic newsletters, regional print and broadcast media.

In the event camp must be cancelled, we will contact the adult email listed above.
In the event of an emergency during camp, we will contact the adult emergency contact listed above.
All children under the age of 18 must be signed in and out at the front desk each day. Children under the age of 12 must be accompanied by an adult when signing in and out. Children 12 and up may sign themselves in and out, but must do so before attending camp.
When you check in your child each day, please include an emergency contact phone number if different than above.
